This works on all pages but please note that the script picks up the url to be submitted from the address bar, so this is NOT a good choice for your home page but it's excellent for individual story pages.

Add the following code to your page or post template where you want the button to appear. The square voting button is shown by default.

To add the button you will need to click on "Edit HTML" and check the radio button that says "Expand Widget Templates".
